1. Counseling for Individuals - Life Transitions:
Participants receive emotional, spiritual, practical, and psychosocial support as they navigate major life transitions such as life-limiting illnesses, job loss or career changes, gender transitions, the breakdown of friendships / relationships, etc. The counselor helps explore identity, meaning, and hope amid change while identifying strengths, resources, and coping strategies. Educational materials and referrals are provided as needed.
One-hour sessions are available in person, by phone, or virtually.
2. Care Partner Counseling:
Services may include, but are not limited to:
Emotional support and validation
Assistance with navigating the healthcare system
Referrals to community resources
Assistance with the long-term care application process
Conversations about Advance Care Planning
Strategies for supporting individuals who are experiencing personal expressions (formerly referred to as "responsive behaviours"), advocacy

3. Grief / Bereavement Counseling:
We experience grief in response to many life transitions. It affects individuals living with dementia and their loved ones as they face loss of health, independence, and imagined futures. We can competently support adults who have experienced any type of loss.
